What a vCard QR code actually contains
The code encodes a vCard — the same contact format your phone already imports. Name, company, job title, phone, email, website and address are all supported; the more you add, the denser the pattern becomes.
For a code that will be scanned from a table tent or a badge, keep it lean: name, organisation, phone, email. For a code on a document people read up close, you can afford the full record including notes.

Why does my scanned vCard save with the wrong details?
Almost always a formatting issue in the source fields — a line break inside a field, or a phone number with spaces and brackets. Re-generate the code with clean, single-line values and scan the printed proof before a bulk print run.
Can a vCard QR code hold a photo?
The format allows it, but an embedded photo makes the payload enormous and the QR code unreadable at normal print sizes. Link to a page with your photo instead, or add the photo to the contact afterwards on the phone.
